Recording Transition Planning Details

Planning for Transition
While transition planning is typically part of the process for students with a support plan (IPP, IEP, BSP, etc.), it can be beneficial to have these details shared for the students who have been a part of our classes for a semester or a year. Within WeCollab, there are two possible areas where this information might be recorded: Student Profile Textboxes or Meeting Notes. As with anything related to Collaborative Response, how it is implemented will be influenced by the context of the school or the district bringing the idea to fruition.
Student Profile Textboxes
Please visit the User Support Site and search "Student Profile Textboxes" for details on how to configure this feature (a functionality limited to those with District Leader or School Leader permissions) and how to use this feature within the Student Profile.
Using the Student Profile Textbox for Transition Planning
Once configured for the Student Profile by a District Leader or School Leader, it will be available in the Student Profile to have its details filled in.
This area is easily accessible for review by all users of the system. A limitation for some users may be that there is a character count restriction on this area as it is intended for brief pertinent details. That being said, it is an excellent area for information to be entered when paired with the "Multiple Student Profile Overview" report (as described in this post) when planning for a group of students.

Using the Meeting Function for Transition Planning
When the Transition Planning meeting descriptor is added by a District Leader or School Leader in the configuration area, it becomes available for selection in the Meeting Descriptor drop down menu.
It can be selected as a choice in an Individual Student Meeting from the Student Profile, if the transition planning is centred on a particular student.
Should transition planning be a collaborative effort where multiple students are discussed, this meeting descriptor is also available for the Collaborative Team Meeting process.

Regardless of the meeting process (individual or collaborative), benefits of this approach include the opportunity to describe the context of the transition needs as well as identify supports to be put into place in the fall for the student(s) and by whom. The notes from the meeting can be emailed to staff to whom the information is applicable (for example, next year's teachers). However, should staff turnover occur, this process does require users to go find the meeting notes pertaining to transition planning.
